This was introduced in Kingdom Hearts Birth by Sleep. This is a trial in which acertain keyblade master chooses a child or an adolescent to inherit the keyblade. If the keyblade also chooses the host, then the host may summon his or her own keyblade at will.
Procedure
A keyblade inheritance ceremony may be intentional or accidental. First a keyblade master must choose a successor. There is actually no required actions or incantations to start this. All the master has to do is test the chosen's strangth by allowing his/her to hold their keyblade, if the keyblade also approves of the succession, it will emit a brief glow of light. Though there are instances that this may be accidental. For example, when a person, or being, with a strong heart and will touched the keyblade of a master, if the keyblade glowed(as shown in birth by sleep when Kairi accidentally held Aqua's keyblade because of her escaping an attack from floods) it may indicate that the person or being is chosen and allowed by the keyblade to summon it.
Prominent wielders to practice this was Aqua and Terra. In aqua's scenario, in Radiant Garden, a swarm of floods attacked Kairi, and because of her innocence and vulnerability, she was scared and hid behin Aqua's back but she accidentally help aqua's keyblade emitting a glow, implying Kairi is then a keyblade wielder. Second was Terra when he visited Destiny Islands, he found the bestfriends Sora and Riku, he sees himself through Riku and he performed the ceremony properly. IT was confirmed that Riku is then a keyblade wielder when the Keyblade chose him. A chosen wielder is capable of seing the door to the heart of a world as riku saw in the flashbacks in kingdom hearts.

- its was revealed in birth by sleep that Sora was not chosen by the keyblade, though this is due to Riku's descent to darkness in kingdom hearts, that led the keyblade to choose to Sora. And possibly that because of Ventus residing in Sora's heart that allowed Sora to wield he keyblade.
- Aqua is currently known to at least chose two person to be a keyblade wielder. Unintentionally, Kairi and Sora. Though it was revealed in an interwview in the ultimania that Aqua was going to let Sora inherit the keyblade, though she felt that Riku was already chosen, and it just reminded her of the future fights that she and Terra will engage, she withdrawn and instead asked Sora to save Riku if he were to fall into darkness.
- Since Sora was never chosen by the Keyblade directly, it is unknown if he will be still able to wield it if Ventus' heart was freed.
- The aformentioned event at Kairi led to support the fact that she indeed is a keyblade wielder and a princess of heart. Due to the fact that she can wield Destiny Place in kingdom hearts 2, it is assumed she is still able to wield it now.

Q5: Why was Riku chosen as the original wielder of the keyblade?
A: Because he was chosen by Terra to inherit.
To use a keyblade you must have been chosen to inherit as well as been chosen by the keyblade itself. In BbS you could see that Terra performed the ceremony with Riku, and that is why Riku was able to take the keyblade that Sora used.

Because he has been chosen to inherit, Riku can see the keyholes of the world.
Q7: Why can Kairi use a keyblade? A: Because she was touched by Aqua’s keyblade.
The keyblade inheritance ceremony is performed with a keyblade master touches someone using a keyblade. When Kairi was running away from the Unversed in BbS she grabbed Aqua’s keyblade, which performed the ceremony. That is why she is able to use a keyblade in KHII to help Sora.
