They are separate "realms" that exist very close together on the same plane. I think of it like the Visible World, Valhalla, and the Unseen Realm from ''FFXIII''. Valhalla = Station of Awakening, a "middle-ground" between life (Realm of Light/Darkness/Sleep) and death (Final World). Or is it that Final World is the next step between life, sleep, and death? I remmeber reading it's a purgatory realm, but either way, "sleep" is a separate place. :::My understanding is that the crossover ''only'' includes the portions where Sora is on his way to the Dive to the Heart, and the surroundings look somewhat like the final world -- basically, the bits with birds and water.
