Donald Duck's Squid Form appears in Kingdom Hearts and Kingdom Hearts II. It is used by Donald while at Atlantica, although it is not used in the Kingdom Hearts: Chain of Memories version of the world, because that is only a facsimile created from Sora's memories.
Donald can still use all of his magic while in Squid Form in Kingdom Hearts, but cannot fight while using it in Kingdom Hearts II, because the battle system was removed from Atlantica in that game.

The Wise Little Hen (1934)
The Royal Court Magician. He's traveling the worlds in search of clues to the whereabouts of the King.
In his previous travels, Donald and his friends helped to defend Atlantica from Ursula, the sea witch.
Appearance
In his Squid Form, Donald's lower body is replaced with that of a mollusk. He has six tentacles, all of equal length, and lacks the two longer tentacles characteristic of squids. The top halves of his tentacles are cyan, while the underside is yellow. Donald does not wear any clothing in this form. Because Donald is a mage, it is likely that Donald's Squid Form is derived from the sea witch, Ursula.
